Schedule: Monday through Thursday clinic schedule. Typical hours 7:00 AM to 5:00 PM with a one hour lunch. No nights. No weekends.

Position Overview

• Employer sponsored primary care and wellness clinic
• Scope includes primary care, acute care, and occupational health or work comp
• Adult focused, with occasional patients under age eighteen
• Proactive care, chronic disease management, early intervention, and patient education
• Work autonomously in accordance with state practice requirements

Required Qualifications

• Active, unrestricted Indiana FNP license
• National certification as a Family Nurse Practitioner
• DEA registration
• Experience treating patients age two and older
• Strong HIPAA and confidentiality standards

Credentialing and Onboarding Items

• Ability to pass on site credentialing
