Primary Care Clinician (MD, NP, or PA) — In-Home Assessments

Location: Florida

Position Overview

• Conduct in-home health and wellness assessments for health insurance plan members.
• Typical responsibilities include history and physical evaluations, blood pressure checks, finger stick glucose testing, urinalysis collection, and distribution of fecal immunochemical test kits.
• Patient population: geriatrics.
• Work is performed independently in members’ homes within assigned counties.
• Documentation is completed in a proprietary iPad-based system.

Schedule

• Monday through Sunday, 7:00 AM to 7:00 PM.
• Typical workload: 5 to 8 assessments per day (assessments generally last 45 to 75 minutes).
• Must have at least one weekday available (weekend-only availability is not accepted).

Required Qualifications

• Active, unrestricted Florida license as an MD, NP, or PA.
• National certification required for NP or PA candidates.
• NPI required (CAQH may be obtained during credentialing if needed).
• Must be willing to travel (minimum 75 miles from home address) and cover a minimum of 3 counties.
• Must carry personal auto insurance and meet driving record requirements.
• Candidates must have at least two years of clinical experience within the past five years to qualify.

• Travel and lodging support may be available based on assignment structure and distance requirements.
