Position Overview

• Provide diagnostic, therapeutic, and preventive care including initial assessment and ongoing management
• Order and interpret diagnostic tests and prescribe medications within scope as outlined in the order requirements
• Educate patients and coordinate care with an interdisciplinary team

Required Qualifications

• Graduate of an accredited nurse practitioner program
• Active Connecticut advanced practice license required prior to start
• Active DEA required
• Connecticut controlled substance registration required prior to start
• Required certifications as outlined in the order requirements
• Candidates must have at least two years of clinical experience within the past five years to qualify
