Primary Care NP or PA

Position Overview
- PACE clinic coverage
- Nurse Practitioner preferred, Physician Assistant considered
- Maternity leave coverage
- Patient visits range from 30 minutes to a few hours
- Patient volume 2 to 5 per day
- Census approximately 247 patients
- Patient population ages 55 and older
- Epic experience is helpful but not required
Clinical Scope
- Comprehensive geriatric assessments
- Ongoing management of chronic conditions including diabetes, hypertension, COPD, and CHF
- Medication reconciliation and deprescribing
- Coordination of care with interdisciplinary team including nursing, PT or OT, social work, and dietitian
- Annual wellness visits and acute care follow ups
- Advanced care planning and goals of care discussions

- Pharmacists
- Occupational and Physical Therapists
- Nurse case manager
- Triage nurse
- Medical assistants
- Community nurses
- LPN
Required Qualifications
- Active Iowa license
- DEA
- BLS
- Medicaid and Medicare number required before presentation
Credentialing and Onboarding Items (must be provided)
- Credentialing timeframe is typically 30 to 90 days
- Required credentialing documents as requested by the client team
